Ordering Xofigo® is a simple 3-step process

Xofigo is ordered and distributed exclusively through Xofigo® Access Services, your one source for ordering Xofigo and treatment support.

THREE WAYS TO START YOUR ENROLLMENT

  • Online: Visit XofigoAccessOnline.com
  • By phone: Call 1-855-6XOFIGO (1-855-696-3446)
  • By fax: Fax completed form below to 1-855-963-4463

If submitting your enrollment by fax, please use one of the following forms:

  1. Standard Fax Enrollment Form
  2. VA Fax Enrollment Form

Whichever option you choose, be prepared to submit the appropriate form, including signed Patient Authorization.

Please have the following information available when you call Xofigo Access Services to place an order:

  • Patient name
  • Patient weight
  • Dosage
  • Facility shipment address
  • Planned date and time of administration

If an insurance benefit verification is needed, submit the Xofigo Access Services Provider Referral Form via the contact information above or directly online.

  • Xofigo Access Services completes benefits investigation, if requested

RECEIVE CONFIRMATION VIA EMAIL OR FAX

  • Provides completed insurance benefit verification and written summary of patient’s benefits, if requested
  • Confirms patient treatment date

NOTE: Xofigo Access Services will fax you a confirmation receipt of your request. If the form is submitted online, you will receive an email confirmation once submitted.

ORDER XOFIGO

After successfully completing your patient enrollment with Xofigo Access Services, licensed authorized administrators of Xofigo can place an order in one of two ways:

  1. Order online: Visit: https://npslogin.cardinalhealth.com/login
    — Unregistered User? Or having issues? Please contact 1-800-495-8832 and select option 2
  2. Order by phone: Call 1-855-6XOFIGO (1-855-696-3446)

If ordering by phone, please call Xofigo Access Services up to 7 days prior to each scheduled treatment date. You can expect the following:

  • Your call will be transferred to Cardinal Health Nuclear Pharmacy Services for actual order placement
  • Cardinal Health Nuclear Pharmacy Services will ship the patient-ready dose to your facility

NOTE: For future doses, Xofigo Access Services will call you to process the order if it has not been placed a minimum of 3 business days prior to the scheduled treatment date.
